What you'll do
- Suitable for those looking for Practice Nurse jobs in Tooting
- Maintain accurate and timely clinical records using EMIS Web
- Deliver person‑centred care and take ownership of your shift
- Provide general nursing care including immunisations, wound care, ECGs, and spirometry
- Administer and check medication safely using MAR/eMAR
- Monitor patients and escalate any concerns without delay
- Manage chronic disease clinics (Asthma, COPD, Diabetes, CHD, Hypertension)
- Maintain a safe, clean, and IPC-compliant environment
- Lead structured and effective handovers
- Coordinate care with families and healthcare professionals
- Support personal care and safe mobility where required
- Oversee patient hydration, nutrition, and comfort
- Supervise and mentor junior team members
- Manage vaccine stock, equipment, and treatment room safety
- Support repeat prescriptions and medication processes
- Conduct clinical audits and contribute to improvements
- Undertake home visits where needed
- Participate in meetings, training, and forums
What you'll bring
- At least 12 months’ experience as a Practice Nurse within the UK
- Active NMC registration (PIN)
- Proven ability to deliver high-quality care in a GP or primary care setting
- Skilled in EMIS Web and maintaining accurate patient records
- Calm, empathetic approach to patient care
- Excellent interpersonal and communication skills
- Able to manage workload independently and prioritise effectively
- Confident handling a range of clinical conditions
- Strong commitment to person-centred care
- Experience supporting more complex patient needs is a plus
