What you'll do

Lead person‑centred care aligned to assessed needs and risk plans

Uphold dignity, independence and the right to choose

Provide personal care, catheter/stoma care within competency, and mobility support

Oversee nutrition, hydration and modified diets (IDDSI where applicable)

Administer medications, complete MAR/eMAR and perform stock counts

Offer therapeutic engagement and emotional support

Maintain safe rooms and communal areas, adhering to IPC

Monitor observations, skin condition and cognition; escalate per protocol

Liaise with families, GPs, allied health professionals and the care manager

Complete accurate records, audits and handovers

Induct, coach and supervise junior staff on shift
