What you'll do

Coordinate person‑centred care delivery, setting priorities for the shift

Support personal care and mobility using safe techniques

Oversee hydration and nutrition; escalate concerns promptly

Administer, check and reconcile medications accurately

Provide companionship and lead meaningful activities

Recognise changes, complete observations and escalate to clinicians

Communicate effectively with families and the MDT

Maintain accurate documentation, handovers and task allocations

Mentor, support and appraise junior carers
