Lead person‑centred care aligned with care plans and risk assessments
Champion dignity, choice and independence
Deliver personal care and mobility support per training
Monitor and encourage nutrition and hydration
Administer medication per policy; maintain MAR/eMAR accuracy
Provide companionship and emotional reassurance
Maintain a safe, clean environment; uphold IPC standards
Observe, record and escalate any clinical or safeguarding concerns
Liaise with families and professionals for continuity of care
Keep comprehensive, contemporaneous records and audits
Support training, supervision and quality improvement on shift
